Here's my list of the best realistic birth and pregnancy mods to improve your game in 2023 and enhance your experience ahead of the Infant Update and Growing Together Expansion! Some of these mods completely change the game and I’d really recommend trying them if you like family gameplay or creating cool storylines.
Woohoo Wellness and Pregnancy Overhaul by Lumpinou
Alright so first up - probably my favourite mod in the list is the Woohoo Wellness and Pregnancy Overhaul mod by Lumpinou. This mod creates really interesting dynamics between family members because you can set a Sims preference for wanting children or not and this preference will have consequences for their relationships and mood. You can also discuss pregnancy and birth control options with other Sims which adds another layer to the intimacy of relationships and choices in the game. If you like stirring up drama, there are also STDs, paternity testing and teen pregnancy which can all cause a little bit of chaos! Something else I really like about this mod is that it includes alternative methods for getting pregnant such as artificial insemination and surrogacy and also allows Sims to have an abortion if they choose to do so. It’s nice to have these options for pregnant Sims and allows for much more complex storylines than the base game. Pregnancies themselves are also explored with much more depth and sensitivity because of new mood swings, cravings and unique emotions. This mod also has a whole bunch of attraction mechanics, but I will leave you to explore that if you are interested!
Child Birth Mod by PandaSama
Another absolutely fabulous mod I’d thoroughly recommend for realism is the Child Birth Mod by PandaSama. PandaSama has so many great mods but this one allows for a much more interesting birth experience. Your Sim will experience contractions and dilation and can perform breathing exercises and other rituals to help the birthing process. I used this mod in one of my old lets Plays that’s now hidden on my channel to do a waterbirth and it was super fun. The experience overall feels more natural than the freaky EA birthing machine. Another great thing about this mod is that your Sim's partner can be heavily involved in the process and help to comfort your pregnant Sim. This means they’re a bit more helpful than usual and won’t be wandering around the hospital and playing on the computer while your Sim’s in labour!
Ultrasound Scan Mod by LittleMsSam
This next one is small but really fun - it’s the Sims 4 ultrasound mod by LittleMsSam. The mod is simple - you can have the option to go and pay to get an ultrasound while your Sim is pregnant. They will disappear for a few hours and come back with a little framed photo. The picture is a nice keepsake and will also reveal information about how many babies there will be and their genders.

Baby Shower Event by KiaraSims4Mods
Another small but impactful mod is the Baby Shower Event Mod. This mod adds a new custom event to the game where you can throw a baby shower and get rewarded with a few extra simoleons and a bassinet. This event can be great for creating drama around the pregnancy or just gathering friends and family together to celebrate.
Stretch Marks Custom Content
Something that I think is really needed in the Sims 4 base game is stretch marks and more realistic skin details. I’ve found a few custom content stretch marks by mooncatcher and saggittariahx which can be added to a pregnant Sim or new parent to celebrate the passing trimesters.

Even More Babies by pekesims (Updated by claudiasharon)
Here's another small one that might be useful if you’re doing a 100 baby challenge! This mod allows for quadruplets, quintuplets and sextuplets by essentially just expanding the maximum number of babies possible in a pregnancy. Using this one of course can get pretty crazy but adds a fun new level of difficulty for challenges and gameplay. You can also choose how likely it is for a sim to get pregnant with multiple babies so that it can be a rare or common occurrence depending on what you’re looking for.
Wonderful Whims by TurboDriver
Okay, so the final one on this list is another pretty huge mod that has a lot of features for both pregnancy and relationships. You might have heard of Wicked Whims before if you’ve downloaded Sims mods, but there’s also a non-explicit version called Wonderful Whims which can be used to enhance a Sim’s pregnancy. Wonderful Whims allows you to change the pregnancy duration and set a risk level for pregnancy even when using the woohoo interaction. Like the Woohoo Wellness and Pregnancy Overhaul mod, it also has its own options for birth control and includes new STDs. Another fun little detail about this mod is that your Sim’s water will break and leave a puddle behind which adds a bit more realism when you have to mop it up.
